Which Services Are Covered By My Insurance?

Different insurance companies have different policies. Please be familiar with your insurance plan and its benefits, as some services provided in our office may not be covered by your insurance at this time. How Do I Pay For My Appointment?

Please contact the office regarding cash prices. The different types of payment we accept are credit cards (Visa, Mastercard and Discover, cash and checks.

Which Hospitals Do You Work With?

We see newborns at Banner Estrella, West Valley Hospital and Banner Thunderbird.

We admit sick patients to Phoenix Children’s Hospital and Banner Thunderbird.

I Am a New Patient, What Do I Need To Bring With Me To My First Appointment?

New patients are asked to arrive 30 minutes early, and bring their Insurance Information, immunization records, and a photo ID with them. If you are transferring from another pediatrician, please request records from them in writing as far in advance as possible.

You may also download our new patient packet from our forms page and fill it out prior to arriving at the practice in order to reduce your wait time. Please visit the forms page to download the forms.

How Do I Request A Medical Release Form?

There is a $25.00 fee for the records requested and printed. Please give us at least an hour of notice. There is no fee if the records are requested directly from new PCP. Please allow 10-14 business days. You need to sign a release form, which you can get from our office or by downloading it from our forms page.
