Please arrive early to a few minutes early to your first appointment to fill out the necessary paperwork. To save time, we have provided you with our patient forms in PDF format for you to download, print and fill out to bring to your first appointment. Forms are available in English and in Spanish.
In order to view these files, you may need Adobe Acrobat Reader. Click Here to download it now. We know your time is as valuable as ours, therefore, unless it is an emergency, we see patients by appointment only and make every effort to stay on schedule.
New patients are asked to arrive 30 minutes early and bring their insurance information, immunization records and photo ID with them. If you are transferring from another pediatrician, please request records from them in writing as far in advance as possible. If you are unable to keep your scheduled appointment, please notify us at least 4 hours in advance.
It is our policy that you call our office at least 24 hours prior to your appointment time. We call to confirm appointments the day before, to give parents an option to cancel their appointment if need be. We understand situations occur that make it impossible for you to make your appointment. If this occurs, please make every effort to cancel or reschedule your appointment in advance.
Please help us keep things running as smoothly as possible. To ensure that patients are seen in a timely manner, it is our policy that failure to notify our office of a cancellation or a need to reschedule an appointment results in a $25.00 charge to your account for each no-show appointment.
Remember when scheduling an appointment at Southwest Pediatrics, it is important to bring your child’s immunization record, insurance card and a photo ID to every visit. Please be familiar with your insurance plan and its benefits, as some services provided in our office may not be covered by your insurance at this time.
Southwest Pediatrics is pleased to welcome the addition of Care Coordinator to our staff. Care Coordination helps families find the right care for children and youth while taking into consideration all aspects of the family-centered life. A Care Coordinator will help in communicating with your healthcare team while focusing on family strength and needs.
